- Identification of the material that has been removed or to which access has been disabled and the location at which the material appeared before it was removed.
- Your contact information, including name, address, telephone number, and email address.
- A statement under penalty of perjury that you believe the material was removed or disabled as a result of mistake or misidentification.
- A statement that you consent to the jurisdiction of the federal court for the judicial district in which your address is located (or in the U.S. District Court for the District of Columbia if you are outside the United States), and that you will accept service of process from the person who provided the DMCA notice.

3. Repeat Infringer Policy
In accordance with the DMCA, we will terminate the accounts of users who are repeat infringers or who engage in activities that violate the copyright laws. If you believe that a user has repeatedly infringed your copyright, please let us know, and we will take appropriate action.
